WASHINGTON BASSET RESCUE "Terms of Adoption"

The following "Terms of Adoption" agreement are entered into this 
_________day of _____________, 2001 between Washington Basset Rescue 
(509) 466-1538 and ______________________ Herein after called "Owner". 
Both parties agree as follows:

Owner agrees to the following:

• The adopted pet is to be a companion and shall not be kept outside as primarily a yard pet._______

• The pet will be leashed or otherwise under control when the pet is outside the home where it resides.__________

• The pet will not be allowed to ride in the open bed of a pick-up truck._____

• The pet must be licensed according to local ordinances, plus must have with I.D. Tags, tattoo or microchip identification.___________

• Pet will be humanely cared for to include food, water, shelter, exercise, any necessary veterinary care, including vaccinations and adequate fencing. _____

• Pet shall not be used for any medical or experimental purpose._____

• Owner shall provide Spokane Basset Rescue with any change of address and phone number.___________

• Owner agrees this pet will not be given or transferred to any other person for any reason whatsoever. If Owner does not wish to keep the pet for whatever reason, the pet shall be returned to Spokane Basset Rescue. Under no circumstances shall the pet end up in any shelter. If the pet is found in a shelter, the Owner is responsible for any costs incurred by Spokane Basset Rescue._______

Owner gives Spokane Basset Rescue permission either directly or indirectly to inquire about or physically examine the pet to determine if the pet is being properly cared for and is in a good, healthy condition._____

Washington Basset Rescue will:

Will provide paperwork verifying pet's spay/neuter, rabies certificate, and immunizations. Every effort has been made by Washington Basset Rescue to evaluate the temperament and health of this pet, but pet often come from a shelter environment and little is known about their past.

If the pet does not work out with the family within _____days, Washington  Basset Rescue will return $__________. After that period, Washington  Basset Rescue will return 
$___________.

I understand that Washington  Basset Rescue is a referral service and is not responsible for the accuracy of the information received about the temperament, habits or physical condition of the pet. I understand that it is my responsibility to see and evaluate the dog for myself before agreeing to adopt it. Washington  Basset Rescue is not responsible for any damage, accident or injury resulting from the placement of this dog in my household.
